If they have one chronic conditions which are expected to last a minimum of 12 months - The doctor must have seen this patient in last 12 months

How does the PCM program work ?
- The care team of the patient will create a care plan , addressing all the unique health needs of the patient, fulfilling its specific goals, manging barriers & enhancing outcomes.
- Each month a dedicated care manager will call the patient to tract the progress & to address any new health issue
- The patient will have access to care coordinator 24/7.
- A copy of care plan will be provided to both provider & patient

- Initial 30 minutes PCM time in a month (Care coordinator's time spent on care plan review, chart reviews, text and phone communication with the patient
- One-Time Billable
- Additional 30 minutes PCM time in a month (Care coordinator's time spent on care plan review, chart reviews, text and phone communication with the patient
- Billable upto 2 times.
