Dr. Mohammad S. Shafi is a board certified Nephrologist and a distinguished figure in the field of Nephrology. He is at the forefront of Care Coordination in Nephrology, blending clinical excellence with a passion for innovation in Value based Care Management. As the visionary co-founder of both the Dialysis Care Center and Kidney Care Center, Dr. Shafi has transformed the landscape of treatment for patients facing chronic kidney disease and renal failure by integrated chronic care solutions that enhance patient outcomes.
He introduced value-based care initiatives to the multi-state Kidney Care Center operation and significantly grew the program. Regularly recognized for his leadership in infection prevention, Dr. Shafi’s influence extends to global health initiatives where he Chairs the Infection Prevention and Control Foundation (IPAC) by driving efforts in preventive healthcare for underprivileged populations. His commitment to improving lives, both in clinical settings and through humanitarian work sets him apart as a pioneering force in patient cantered and value based care.
Dr. Babajide Salako is an experienced health care leader with a diverse background in medicine and healthcare management. His expertise lies in leveraging his business acumen to drive the company’s growth and operational success. He served as the Senior Director of Disaster response and Business continuity at Fresenius, managing and responding to disasters affected kidney patients worldwide.
Since becoming Chief Executive Officer of Dialysis Care Centre and Kidney Care Center in 2014, Dr. Salako has been responsible for overseeing both of the company’s expansion and ensuring the delivery of high-quality patient care. He spearheaded the integration and of value-based initiatives throughout the multi-state Kidney Care Center operation and its’ nationwide patient base. Dr. Salako holds an MBA from George Washington University and is a physician trained in Internal Medicine.
Salman Azam is an experienced attorney and executive in the healthcare space. During his 15 years of private practice, Salman represented several medical and dental specialty groups across the country in transactional, regulatory, compliance, employment, financial and business-related matters. He has also counseled several healthcare billing and credentialing companies with respect to C.M.S. and insurance related issues.
He joined Dialysis Care Center and Kidney Care Center as their General Counsel and Chief Compliance Officer in 2018 where he was instrumental in the Companies’ multi-state expansion efforts and assisted the C.E.O. in the implementation of Companies’ initiatives including Value Based programs. Salman was a named a Notable General Counsel by Crains Business Magazine for his work with Dialysis and Kidney Care Center and was featured as a TEDx speaker as well.
